This form is designed to collect comprehensive biographical information of the Proctor/Student Welfare Chief or other personnel assigned this responsibility, in coordination with the Principal/Campus Chief, from each Tribhuvan University central/constituent campus. The form also aims to assess the existing sports infrastructure, evaluate its utilization by campus students and the surrounding community, and identify future needs and recommendations.
